This encompasses the complete life cycle of a standards specification—development, adoption, market recognition, utilization, and adherence. The HL7 specifications are unified by shared reference models of the health care and technical domains. The HL7 version 2.4 messaging standard is currently in use, and version 3, which represents several fundamental changes to the HL7 messaging approach, is in an advanced stage of development.
Many people know of HL7 as an organization that creates health care messaging standards. This article is intended to serve as an introduction to the CDA standard. It is geared toward medical informaticians who do not have significant familiarity with HL7 version 3, and it is intended to introduce the approach and objectives used in the creation of the standard and present an overview of the standard—not sufficient for implementation but sufficiently detailed to enable the reader to understand the scope and contents of the standard. The need for a clinical document standard stems from the desire to unlock the considerable clinical content currently stored in free-text clinical notes and to enable comparison of content from documents created on information systems of widely varying characteristics. Given the variability in clinical notes, including structure, underlying information models, degree of semantic encoding, use of standard healthcare terminologies, and platform- and vendor-specific features, it is currently difficult to store and exchange documents with retention of standardized semantics over both time and distance. And while the current CDA standard does not fully enable these main driving objectives, it takes us a step closer.
The CDA is a document markup standard that specifies the structure and semantics of “clinical documents.” A clinical document is a documentation of observations and services and has the following defining characteristics:

Persistence. A clinical document continues to exist in an unaltered state, for a time period defined by local and regulatory requirements.

Stewardship. A clinical document is maintained by a person or organization entrusted with its care.
*
Potential for authentication. A clinical document is an assemblage of information that is intended to be legally authenticated.

Wholeness. Authentication of a clinical document applies to the whole and does not apply to portions of the document without the full context of the document.

Human readability. A clinical document is human readable.
Many draft and existing standards have helped inform the development of the CDA,14 and several guiding principles have driven the design:

There are many requirements and uses for clinical information, such as direct patient care, outcome research, and public health reporting. The CDA will give priority to defining documents that are created by clinicians involved in direct patient care, assuming the other uses will be derivable. Minimize the technical barriers needed to implement the Standard. There are estimated to be hundreds of thousands of nonstandardized clinical documents in existence. The CDA will facilitate standardization of these documents by allowing cost effective implementation across as wide a spectrum of systems as possible; by supporting exchange of human-readable documents between users, including those with different levels of technical sophistication; by enabling a wide range of post-exchange processing applications; by providing compatibility with a wide range of document creation applications; and by using non-health-care–specific standards where possible.

Promote longevity of all information encoded according to this architecture. The CDA documents will be application- and platform-independent and can be viewed and edited by a number of tools, both now and in the future.

Promote exchange that is independent of the underlying transfer or storage mechanism. The ability to exchange or store CDA documents will be application- and platform-independent. These documents can be exchanged in HL7 messages, via e-mail, on a floppy disc, etc. A CDA document can be stored as an independent file, within a document management system, within a database, etc.

Enable policy makers to control their own information requirements without extension to this specification. The CDA will define an extensibility mechanism that allows local implementations to represent information that is not formally represented in the standard.

This family, which includes both CDA and the evolving version 3 message standards, all derive their semantic content from the shared HL7 Reference Information Model (RIM)15 and are implemented in Extensible Markup Language (XML).16 The process for generating an XML-based implementation from the RIM is part of the version 3 development methodology. The exact style of HL7’s XML representation was a carefully considered balance of technical, practical, and functional considerations.18–
At the heart of the HL7 version 3 family of standards lies a shared information model, the HL7 RIM.14 An object-oriented model created as part of the version 3 methodology, the RIM is a large pictorial representation of clinical data and identifies the life cycles of events that messages and documents convey. The RIM includes a new set of data types developed for use in the HL7 version 3 family of standards. These data types include some of the familiar ones used in HL7 v2.x messaging, such as STRING (ST), INTEGER (INT), and TIME STAMP (TS), as well as new data types such as ENCODED DATA (ED), which supports multimedia; INTERVAL of TIME (IVL<TS>), which allows for the expression of a time range; and CONCEPT DESCRIPTOR (CD), which supports the post-coordination of codes (or, stated in another way, the combining of codes from a terminology to create a new concept).

This architecture, which can be thought of as a set of hierarchically related XML Document Type Definitions (DTD) or schemas,24,25 is envisioned for future releases of the CDA standard. The current CDA standard has defined only the top node of the hierarchy, known as “CDA Level One.”
Level One is sufficiently detailed to represent largely narrative clinical notes. The specification defines the document header in detail, while the document body is largely structural. Level One is intended to minimize the technical barriers to adoption of the standard while providing a gentle introduction to the RIM. It is hoped that the provision of deeper levels of the architecture will provide a migration pathway for implementers to iteratively add greater markup to clinical documents.
Level Two is envisioned as a set of templates or constraints that can be layered on top of the CDA Level One specification. A template may, for instance, specify that a document of type “history-and-physical” will contain a mandatory “subjective” section; a mandatory “physical-examination” section containing a mandatory “vital-signs” section and an optional “cardiovascular-examination” section; and a mandatory “assessment” section followed by a “plan” section. This type of template development will require considerable domain knowledge and participation from professional societies so that created templates are widely embraced and supported. Health Level 7 is now in the process of establishing necessary liaison relationships with professional societies.
Level Three of the CDA will add additional RIM-derived markup to the CDA Level One specification, enabling clinical content to be formally expressed to the extent that is it modeled in the RIM or to the extent that it can be expressed in an HL7 version 3 message. This could, for instance, allow an order message to be extracted from a clinical document, allow for a detailed representation of symptoms and findings, and allow for billing codes to be automatically extracted.
The clinical content of CDA documents will remain invariable across all levels of the architecture. Each level both enables and enhances the standardized expression of richer shared semantics. Thus, a single report can be marked up as a Level One, Level Two, or Level Three document, and its clinical content will not vary. What will vary between the levels are the degree to which clinical content can be machine processible in an exchange context and the degree to which clinical document specifications can impose constraints on content.Clinical Document Architecture Technical Overview

The CDA body comprises sections, paragraphs, lists, and tables. These structural components have captions, can nest (so that, for instance, a section can contain a section), and can contain character data, multi-media, and codes drawn from standard terminologies.
Appendix 1 shows a sample clinical document, and Appendix 2 shows a CDA-encoding of that sample document. What this means, practically speaking, is that instead of making all the Mac and iOS devices in your network run out over the Internet to download and update content from Apple’s servers, you can.
I’ve been working with Mac hosting for about thirteen years. I tell people about using a Mac mini as a server and they just love the idea of it. There are a lot of great hosting options out there these days, but it’s nice to work with a server where the OS is familiar and you know you control the whole stack of hardware and software. Our base Mac mini subscription starts at just $79/mo (that includes the server) so it’s much easier to get started.
Five years ago today, I wrote a post about ways you might use a Mac server. I’ve decided to update that list and add some new ones that have come along in the last little while. It’s a mix of Mac apps, open source projects, and iOS tools. Ways to use your Mac Server
macOS server is in the middle of a big transition, but I’m still including it here. You’ll want to be sure to read up on the future roadmap. It’s a first party way to run a Profile Manager and Open Directory.
There are a number of ways to use your Mac as a build server. It’s easily the most popular use here at MacStadium. You might look at Jenkins, Buildkite, Teamcity, Veertu, or Gitlab.

I’ve written about the Mac server I keep in my house so many times that I sometimes forget that people don’t keep a catalog of everything I’ve written in a database somewhere, tagged by topic.1 So when I wrote about using a hacked Intel NUC as a replacement for my Mac mini server last week, I got a bunch of questions about what I was using my server to do.
Fair enough. I’ll explain, but first let me tell you a little about the history: I’ve been running a Mac server since before there was a Mac mini, back when I first got a dedicated Internet connection for the first time. (It was DSL, and for the decade I had it, it went from being miraculously fast to horrendously slow.) My first server was a beige Power Mac G3 I picked up from an employee sale at Macworld, and it was replaced by my Power Mac G4 when I migrated to a G5. The G4 was replaced by the first Mac Mini, then an Intel Mac Mini, and finally by a Core 2 Duo model.
In the mid-90s I co-wrote a book about using Mac OS as a server, so the moment I could have a server in a closet in my house, I did it. In the early days I used it as a web server (for InterText, my fiction magazine. Money works for mac. I used FileMaker to build a few web databases, including a home-built app that let me run a fantasy football draft before there were web-based tools that let you do that.
I also ran my own email server for a while, using MailShare (later Eudora Internet Mail Server), a remarkably robust Mac email server. In the end I gave up and switched to Gmail for all of my mail, not because MailShare let me down, but because the sheer volume of spam connections to my mail server were swamping my slow DSL connection. (Plug for an occasional podcast sponsor: If I had been using MailRoute to pre-screen my inbound mail, I could’ve kept running it.)
Over the years all of those uses fell away, but new ones replaced them. For years it’s been really convenient to have an always-on computer somewhere on my local network, attached to a large hard drive (or multiple hard drives), containing my media library. In the early days, that meant thousands of MP3s. iTunes and the Slim Devices media server meant that I could play anything from my music collection on a laptop or my stereo. These days my music streams over the Internet, but I’ve still got a sizeable collection of video files that are served up to my various devices via the Plex server. I also store a lot of large archival data sets—old podcast projects, mostly—on my server.
In 2004 I set up a weather station in my backyard; to get that data on the Internet, I needed to attach a radio receiver to my Mac via a PC serial-to-USB adapter. It worked, but it was janky and required the computer to be close enough to the weather station to receive its signals. At some point, the manufacturer of my weather station offered a new add-on module that gave the station’s indoor display console an Ethernet port and automatically uploaded data to the cloud.
WeatherCat, the Mac weather-station software I use, can talk to that module directly, so my computer no longer needs to be within radio range of the weather station itself. While I could just rely on cloud services like Weather Underground to display my weather data, I’ve built up a large database of historical data—all generated by WeatherCat and served by my server using its built-in web server. So I keep using WeatherCat, and keep promising myself to update the web templates I created 15 years ago.
Late last year I started running HomeBridge on my server, which allows non-HomeKit devices in my house to be visible to Apple’s Home app. It has worked quite well, and it’s awfully nice to have a single unified interface for all the smart devices in my life.
As for the many large hard drives I used to have attached to the server, a few years ago I replaced them all with a Drobo 5D (disclaimer: it’s another former podcast sponsor), which is an even larger hard drive, with some added redundancy in case of a drive failure. (A few drives have failed; so far, all I’ve had to do is pop the dead drive out and pop in a fresh one. I keep a spare drive or two around just in case of failure, and yes, I do back up all the important data to the cloud, because RAID is not a backup.)
So, long story short, today my server is acting as a file server, weather station, HomeKit bridge, and web server for a few miscellaneous files. Could I do the same thing with a NAS box, or if I just installed Windows 10 or Linux on that Intel NUC instead of hacking it to run High Sierra? Sure, but I am vastly more comfortable with macOS. And as someone who tends to travel without a Mac, sometimes I find my self in desperate need of one. Most often, someone hands me a file—usually a QuickTime movie recorded by Call Recorder—that I just can’t process on iOS. I can use an app like Screens VNC to connect securely to my home server, drag the file from my Dropbox folder and drop it onto a conversion utility, and then disconnect. The converted files go right back into Dropbox, and I return to my iOS workflow. I know, it’s cheating—but it’s awfully useful to have a Mac on call if you run into a brick wall in iOS.

You can create a bootable drive to store an OS X Recovery Disk so that you can access recovery tools anytime, anywhere. It would be great if you could invest in a high-capacity USB 3.0 flash drive to store the required tools.

In order to use it, however, you’ll need to get to grips with its basic commands and functions. Once you’ve done that, you can dig deeper and learn more commands and use your Mac’s command prompt for more complex, as well as some fun, tasks.

There are a few things you need to bear in mind when you’re typing commands in Terminal, or any other command-line tool. Firstly, every character matters, including spaces. So when you’re copying a command you see here, make sure you include the spaces and that characters are in the correct case.
You can’t use a mouse or trackpad in Terminal, but you can navigate using the arrow keys. If you want to re-run a command, tap the up arrow key until you reach it, then press Return. To interrupt a command that’s already running, type Control-C.
Commands are always executed in the current location. So, if you don’t specify a location in the command, it will run wherever you last moved to or where the last command was run. Use the cdcommand, followed by a directory path, like in Step 1 above, to specify the folder where you want a command to run.
There is another way to specify a location: go to the Finder, navigate to the file or folder you want and drag it onto the Terminal window, with the cursor at the point where you would have typed the path.
Here’s another example. This time, we’ll create a new folder inside your Documents directory and call it ’TerminalTest.’

Open a Finder window and navigate to your Documents folder.

Type cd and drag the Documents folder onto the Terminal window.

Now, type mkdir ’TerminalTest’
Go back to the Finder, open Text Edit and create a new file called ’TerminalTestFile.rtf’. Now save it to the TerminalTest folder in your Documents folder.
In the Terminal window, type cd ~/Documents/TerminalTest then Return. Now type lsand you should see ’TerminalTestFile’ listed.
To change the name of the file, type this, pressing Return after every step:

That will change the name of the file to ’TerminalTestFile2’. You can, of course, use any name you like. The mv command means ’move’ and you can also use it to move files from one directory to another. In that case, you’d keep the file names the same, but specify another directory before typing the the second instance of the name, like this:

To delete all the files and sub-folders in a directory named ’oldfolder’, the command is rm -R oldfolder and to confirm each file should be deleted, rm -iR oldfolder
Just because you can use Terminal to delete files on your Mac, doesn’t mean you should. It’s a relatively blunt instrument, deleting only those files and folders you specify.Another way to free up space

With MoneyWorks, you have the choice of buying a licence, or subscribing to one, or a combination of both.
MoneyWorks subscriptions enable you to pay a low, monthly fee for continued use of MoneyWorks. Here’s how it works:
*Create or login to your MoneyWorks account
*Register your Visa/Mastercard credit or debit card details1
*Choose the product to which you wish to subscribe
*Agree to the Terms and Conditions
*Your credit card will be charged for the first month’s subscription, and a MoneyWorks activation code will be sent to you
*Every month from that date, your credit card will be automatically charged for the next month’s subscription.
*To modify or suspend the subscription, simply log into your MoneyWorks account
A subscription will cover any updates and upgrades released while the subscription is current, but does not include support (which will be charged separately).
Note that, with subscriptions, MoneyWorks will occasionally check back to our server to check your subscription status. For this reason you need to have internet access every ten days or so.
If you suspend your subscription, or otherwise don’t pay, your MoneyWorks will stop working. Simply renew the subscription for continued access.
